They assess, plan and deliver timely interventions for patients who have mental health problems and are over the age of 16. They also provide training to hospital staff on a range of topics relating to mental health

The Older Adult Liaison Team aims to provide a high quality service for older adults in an acute care setting. Patients referred to the team may be suffering from low mood or anxiety. The team also assess and support patients who have a diagnosis of dementia or who appear to be suffering from a degree of cognitive impairment.

The team maintains good links with older adult community services and can refer to into memory services if appropriate.

The team also work alongside discharge teams at County Hospital to support appropriate and timely discharge for our patients.
