For your staff record you need to make your request to your Line Manager or contact the HR Department
For Data not in held in your health or staff record
Quite often, patients and staff members request personal data information (such as e-mails held on the Trust servers) which do not form part of your health or staff record, for example, where this request is made as part of a Complaint or a general Subject Access request, the relevant team will liaise with the Information Security team.
