New Online Portal – Patients Know Best (PKB)

A single place for your health information
Access via the NHS App using your NHS login

Patients Know Best is UHNM’s free online patient portal which provides you with access to your personal health information and designed to improve your patient experience.  PKB will enable you to access NHS services and information via a website you can access from any device, and the NHS App using your NHS login and give you more choice and control over your care.  All your information is securely stored in one place with instant notifications when new information is made available.

News update: Digital letters are now live in the patient portal. Since early June 2023 those already registered (or registering) for PKB will now receive some of their letters digitally within the portal. If you read these letters in the portal before your appointment date a paper letter may not be sent to your address. For urgent appointments within 7 days the trust will always post a letter to your address.

You will receive an invitation to register for PKB via email, SMS text message or posted letter. If you have not received an invitation but you have been a patient recently, you can register via the link below, once registered you can access PKB via the NHS App.

After registering you can use PKB via the NHS App.

The benefits of using PKB are:

  • Accessing your appointment information digitally; including your outpatient or inpatient appointment letters. 
  • Accessing other types of letters including appointment booking letter invites, referral acknowledgements, DNAs  & other clinic letters. 
  • Pathology and radiology results will be available in the future including messaging, questionnaires and care planning.
  • Personal information can be shared with your family, carers and health professionals including pharmacy. This will be your choice and can be amended at any time.
  • You can view your medical information which will include emergency department, inpatient and outpatient visits.
  • You can record your medical information such as any allergies or medication you are taking.
  • Diagnosis information and information relating to devices you are using such as fitbits.
  • Track and monitor symptoms and access resources specific to you added by your healthcare team.
  • Total access either from home or abroad.
  • Option to keep a health journal.
  • Radiology letters and appointments are planned for later in 2024.
  • Other letters including hospital discharge, ED discharge and other clinical correspondence are planned for later in 2023.
  • Pathology and radiology results will be available in the future including messaging, questionnaires and care planning.

You will be sent an email to the email you provided when you registered.

You will see the last 2 years of appointments or visits. Currently this does not include Radiology appointments or letters.

Also some letters including; outpatient and inpatient appointment letters, appointment booking letter invites, referral acknowledgements, DNAs & other clinic letters are also available now. The trust is working hard on adding more letters into the portal such as radiology appointment letters, hospital discharge, ED discharge and other clinical letters during 2024.

Letters which are not read in the portal will be sent out via post.  If the appointment is within 7 days  you will always receive the letter in the post.

You can add your own information if you wish, such as tracking your symptoms or recording medications you are taking.  You can also keep your own notes to monitor your condition.

Soon in 2024, inpatient and outpatient appointment letters will be added in the portal. Letters which are not read in the portal will be sent out via post. You will have the option to opt out via the trust PKB support email below. If items such as hearing aids or leaflets need to be sent out with your appointment, these will continue via post.

These will be available during the second phase of the PKB project during 2024-25 for Pathology and Radiology.

Yes. The PKB software meets NHS requirements for health record systems and is fully compliant with the General Data Protection Regulation (GDPR).

Only the minimum level of data necessary to verify you has been uploaded. All personal and health information stored is encrypted.

Yes as this is an online service and will require you or, a carer, to have regular internet access to receive your letters and use any additional features

Yes. you can only use one unique email address per registration e.g. you can’t use a family shared email if your family/friend already uses that email for their PKB account.   If you are registering via the NHSApp, it will automatically use the email address associated with that account.

If you want the family member or carer to have access to your portal record they can be added in the ‘Sharing’ section. This will allow them to set up their own account and allow you to control the level of access they have to your record. For more information click here.

Some patients may not have any information in their records when they first register however this will change over time as you visit the hospital for treatment.  You can still add your own information, such as tracking your symptoms or recording any medication you take.

Contact the Trust by emailing pkb.uhnm@uhnm.nhs.uk to change your information.  This will then be updated in the portal when you are next booked an appointment.

 

No, there is no cost to you. Using the portal will help us to reduce costs such as paper and postage which can be reinvested to provide better care.

For patients who visited UHNM within the last two years (before Jan 2023), your historical appointments will be available in PKB.  Any future appointment information will show immediately in PKB when an appointment has been booked.  Unfortunately Radiology appointments are not yet included but coming soon in 2024.

This is your choice if you want to interact with the hospital in a digital way and requires you to ‘opt in’.  Ignore the emails inviting you to register if you choose not to use this method.
