At UHNM (University Hospitals of North Midlands), we want to give you the best care and keep you safe. To do that, we need to know some things about you.
Children
We might collect:
• Your name, birthday, and address – so we know who you are and where to send letters
• Your NHS number – a special number just for you
• Your doctor’s name – so we can talk to them about your care
• Who brings you to your appointments – to help keep you safe
• Notes from doctors, nurses, or social workers – to help us understand how to care for you
• Pictures or videos – only with you and your parent’s permission
• Device location tracking (GPS) – if you are given certain medical devices to provide you with treatment and care the location may be tracked by your care team. You will be told if you have one of these devices.
We get this information from you, your family, your doctor, or others who help look after you.
We keep your information private. But sometimes we need to share it with people who help care for you, like:
• Your family doctor – to help with your care
• Your school – if they need to support you
• Social workers – if they’re helping you or your family
• NHS 111 – if you call for help or advice
• Other health experts – we only share what’s needed, and not your name unless it’s important
We only share what’s needed to help keep you safe.
Your health record is a private file that includes everything we know about your care. You can ask to see it, and we’ll explain anything that’s hard to understand.
If something looks wrong, tell us so we can fix it.
Sometimes we need to share information to protect you or someone else. This helps us:
• Follow the law – we have rules to follow that help keep everyone safe
• Work with people like social workers or police – they help protect children and families
• Make sure you’re safe and well – so we can look after you properly
We keep your information safe for as long as we need to, depending on what happened.
You can ask to see your information. But sometimes we might not be able to show everything if it could hurt someone or stop us from helping.
If you’re worried about how your information is used, you or your parent can talk to:
• PALS: patientadvice.uhnm@nhs.net they help with questions and concerns
• Complaints Team: complaints.department@uhnm.nhs.uk if you want to make a formal complaint
• Data Protection Officer: dspuhnm@uhnm.nhs.uk makes sure your information is looked after properly
• Information Commissioner’s Office: 0303 123 1113 – only contact them if you're still not happy after speaking to us
