Special Chemistry
Special Chemistry testing involves complex laboratory techniques that require a high level of staff expertise for both performing the analyses and interpreting the results. Many of these tests have extended turnaround times due to the specialist preparation, batching, and interpretation involved.
Tandem Mass Spectrometry
The department operates a tandem mass spectrometer, configured for the analysis of:
• Tacrolimus / Cyclosporin
• Urine metanephrines
• 5HIAA
These tests are performed in batches, with a typical turnaround time of approximately two weeks. Due to the extensive preparation required, it is not possible to provide urgent analysis for these tests. In some cases, high demand for certain analytes can result in backlogs.
Protein Electrophoresis & Immunofixation
The department also performs protein electrophoresis and immunofixation on blood and urine samples for the diagnosis and monitoring of patients with myeloma and other plasma cell disorders. These are technically demanding analyses that require considerable experience to interpret, and may require follow-up investigations. A turnaround time of at least two weeks should be expected.

Special Chemistry services are available to all NMCPS locations — Royal Stoke University Hospital, County Hospital, Leighton Hospital, and Macclesfield Hospital — with specialist analyses centralised for consistency, quality, and expertise.
