Supporting households with children diagnosed with asthma
Keep Warm, Keep Well
The NHS in Stoke-on-Trent and Staffordshire is working in partnership with Staffordshire charity Beat the Cold to provide additional support to households with children diagnosed with asthma, whose health could be made worse by living in a cold home.
The One Health and Care record is being used to identify patients with diagnoses who live in households that are likely to benefit from support provided by Beat the Cold.
This project is funded by the Keep Warm, Keep Well community energy scheme, delivered by a partnership between the University Hospitals of North Midlands (UHNM) and Staffordshire Community Energy (SCE).
Your Referral Offer
As part of the NHS in Stoke-on-Trent and Staffordshire’s work to proactively help some of our patients, with certain health condition diagnoses and/or vulnerability to keep well, you have been identified as likely to benefit from support provided by Beat the Cold.
This is because of your medical condition diagnosis and/or vulnerability and information based on where you live. Information has been provided by the One Health and Care record.
What Help Will I Receive?
A Beat the Cold Energy Adviser will conduct a review and, if necessary, a home visit. The time and date of this meeting will always be agreed with you in advance. All staff are DBS checked and carry identification.
During the call / home visit the advisor will:
- Identify any benefits that you may be entitled to and refer you to agencies that will support you to access these.
- Identify ways in which you can safely reduce energy use in your home, while maintaining a safe indoor temperature.
- Identify any funding that is available to make your home more energy efficient, and support in applying for funding if requested.
- Help you to resolve issues of fuel debt, or any other problems that you may be having with your energy supply or supplier.
- Register you as a priority customer with the National Grid.
Your participation in the referral and Beat the Cold intervention will be shared with the NHS in Stoke-on-Trent and Staffordshire. This will allow the impact of the scheme to be measured.
How do I take up this offer?
You do not need to do anything.
Beat the Cold will contact you via telephone.
A specialist advisor will provide further information about your Keep Warm, Keep Well referral from The NHS in Stoke-on-Trent and Staffordshire. They will answer any questions and arrange the next steps if you wish to take up this FREE service.
