University Hospitals of North Midlands shortlisted for the 2023 HSJ awards
University Hospitals of North Midlands has been shortlisted for two HSJ Awards which recognises the outstanding efforts and achievements that teams make to improve patient care.
A pilot project which identifies patients from UHNM’s waiting lists who meet the criteria for a weight management programme has been shortlisted for the Data-Driven Transformation category, while the Acute Hospital at Home team has been shortlisted in the Primary and Community Care Provider of the Year category for its Urgent Care Co-ordination Centre and Community Rapid Intervention Service (CRIS).
The Automated Weight Management Referrals project identifies patients from the Trusts’ waiting lists who meet the weight management programme eligibility criteria and asks for referral consent though the use of IT technologies.
Andrew Fraser, UHNM Business Intelligence Manager, said: “On behalf of all the great team involved in this pilot Weight Management Referrals process project, the news of our shortlisting for the HSJ Data-Driven Transformation Award is amazing and we’re all absolutely thrilled.
“UHNM has great staff and talents and when working together we can do create amazing work. Our entry for the awards is a fully automated process for referring patients who meet eligibility criteria into the pilot NHSE weight management programme.
“We’re one of a few trusts involved in the pilot and we’re the only trust with an automated approach. This is working brilliantly and we’re topping the charts for referrals with minimal staff overheads in achieving this.”
The Community Rapid Intervention Service (CRIS) is an integrated service provided by both UHNM and Midlands Partnership NHS Foundation Trust (MPFT) for patients at risk of needing an admission to hospital and the Urgent Care Co-ordination Centre provides easy access to diagnostic tests and review by hospital consultants in one place and aims to prevent unnecessary hospital admissions and provide a better experience for patients.
Dr Zia Din, Deputy Medical Director and Chief Clinical Information Officer at UHNM, said: "We are delighted that our Acute care at home team, a collaborative service across the ICS are shortlisted in the Primary and Community Care Provider of the Year category at the HSJ Awards for their CRIS and Urgent Care Co-ordination Centre work which prevents unnecessary hospital admissions by providing care at home for our patients resulting in a better patient experience and less pressures on the hospital."
