Elderly care and frail elderly
The elderly care wards are: 76b, 78, 79. Ward 76b has 21 male acute elderly care beds. Ward 78 and 79 have 27 female elderly care beds.
The wards provide on-going acute medical support and work towards a timely and safe discharge for patients. This may be to community home based services or bed based services, dependent on the patient's needs. Each ward has a daily consultant ward round during the week and the Frail Elderly Assessment Unit (FEAU) has a full ward round at the weekends and on Bank Holidays.
Wards 76b, 78 and 79 have a visit from the on call geriatrician at weekends and Bank Holidays to support discharge decisions and manage acutely unwell patients.
Six consultant geriatricians participate in the acute medical rota in the evenings and weekends and consultants provide a referral service for older people on all medical and surgical wards.
These services are provided by Staffordshire and Stoke on Trent Partnership Trust within the community from Bradwell, Longton, Haywood, Leek and Cheadle hospitals.
The consultant geriatricians work closely with community services to ensure excellent quality care for elderly patients. Assessment services are also offered at local residential and nursing home facilities.
Our Geriatricians work collaboratively with colleagues in intermediate care and rehabilitation services to ensure patients receive the best care possible. (Intermediate and rehabilitation services are managed by Staffordshire and Stoke on Trent Partnership Trust).
Each ward has a team of staff which includes Consultant Geriatricians, Specialist Registrars, Speciality Doctors, Foundation Doctors, GP Trainees, Core Medical Trainees, Advanced Nurse Practitioners, Senior Clinical Nurses, Physiotherapists, Occupational Therapists, Speech and Language Therapists, Dieticians and Pharmacists, Lead Nurses, Specialist Assessment Nurses and Social Workers. Some staff are dedicated to a specific ward and are thus able to provide a continuous and seamless service for the patient.
The Frail Elderly Assessment Unit is based at the front of the West Buildings. It is an emergency assessment unit specifically provided for frail older people with an acute medical problem.
Admission to this area is via referral from the emergency department, community services (for example, the patient's GP or a Community Matron) or West Midlands Ambulance Service.
The geriatricians provide specialist services which include:
- A 'falls clinic', which is currently situated at Haywood Hospital
- Orthogeriatric liaison on Ward 225 for patients with fractured neck of femur
- Outpatient clinics at Royal Stoke University Hospital, Leek, Cheadle and Haywood Hospitals
This service is provided by North Staffordshire Combined Healthcare. There are memory clinics at Cheadle Hospital, Harplands Hospital, Bradwell Hospital and Abbott's House at Abbey Hulton.
There is an acute assessment team and a hospital based complex needs team for EMI (Elderly Mental Infirm) patients. All elderly care wards have access to these services. The geriatricians work closely with the old age psychiatry team to support the care of older people with memory problems, depression, dementia and challenging behaviour.
Ward 15 is the Elderly Care ward, located on the Second Floor. Patients are also cared for on part of Ward 11.
