We aim to facilitate the delivery of NICE CG83 – Rehabilitation after Critical Illness guidance. This encompasses the assessment of patients at high risk of physical and non-physical complications through the short clinical assessment. Patients deemed at risk go all then receive comprehensive assessments to determine their rehabilitation needs which will lead onto individualised goal setting and appropriate referrals to specialist services.
Our current service follows the patients throughout their Critical Care stay to the ward, discharge home where they are able to access further advice and support via telephone post discharge.
Further clinical follow-up is awaiting funding approval.
Our aim through early mobilisation is to promote a return to each patient’s personal preadmission physical baseline. Through patient centred goals, treatment promotes independence and a return to normal function. Where possible families are included to meet individual patient needs and assist with setting goals.
Our physiotherapy and occupational therapy which has enabled us to follow patients through to the ward to provide a more seamless service. A 6 week follow up therapy clinic is also supported by the increase staffing to further promote patient’s independence.
